Part time RN - Home Care

Job Summary:

The Registered Nurse (RN) is responsible for providing supervision of care in alignment with each client’s individualized plan of care. This role involves conducting face to face in home assessments, developing and updating care plans, ensuring service adequacy, and providing education and oversight to care staff of CNA’s and PCA’s.

Coverage Area:

· Carroll, Coweta, Fulton, Fayette, Heard, Spalding, Troup, and Upson

Primary Responsibilities:

• Conduct initial assessments for new clients.
• Perform assessments for existing clients experiencing changes or interruptions in service.
• Develop, review, and update service plans/care plans.
• Determine medical frailty status for new and current clients.
• Conduct supervised visits with current clients every 92 days to evaluate service adequacy and updates.
• Provide education, guidance, and oversight to care staff to ensure high-quality care.

Skills & Qualifications:

• Ability to utilize a laptop or tablet to input information into Wellsky and collect client signatures electronically.
• Enter all medications into Wellsky within the Care Plan.
• Maintain all required certifications, including CPR, First Aid, and TB testing.
• Complete additional training as required by the organization.
• Possess a valid driver’s license and have reliable access to a vehicle.

Pay: $45.00 per hour
