Qualifications:
- RN (Required)
- 5 Years experience (Preferred)
- Familiar with CAHC Accreditation Standards
Job Responsibilities includes for includes:
- Performs admissions at a patient's residence
- Initial client assessments
- Write plans of care
- 30 day calls
- 60 day revisits
- Case orientations
- Clinical employee field supervisions
- Complete competency review
Job Type: Part-time
Pay: From $40.00 per hour
Expected hours: 2 – 10 per week
