Position Overview:
We’re seeking a licensed Occupational Therapist to join our team on a per diem basis. You'll conduct in-home visits to assess, treat, and guide patients toward greater independence in daily living.
Key Responsibilities:
- Perform home-based OT evaluations and develop care plans
- Provide hands-on treatment and therapeutic interventions
- Educate patients and caregivers on exercises, equipment use, and safety
- Maintain accurate and timely documentation
- Collaborate with interdisciplinary team as needed
Qualifications:
- Current Occupational Therapy license (State of [insert state])
