Position Overview:
We are looking for a Licensed Practical Nurse (LPN) to support our growing Class A home care agency. The LPN will play a vital role in providing exceptional care to our clients, including medication management, wound care, oxygen therapy, and coordination with healthcare providers. The ideal candidate is a caring, detail-oriented professional who is passionate about working with older adults and ensuring they receive the highest standard of care.

Key Responsibilities:
- Provide direct patient care, including medication administration, wound care, and oxygen therapy.
- Assist in developing and updating personalized care plans based on the individual needs of clients.
- Collaborate with Registered Nurses (RNs), caregivers, and healthcare providers to ensure the best possible outcomes for clients.
- Monitor client conditions and report any changes in health or well-being to the supervising nurse or physician.
- Educate clients and their families on proper care techniques, safety precautions, and the use of medical equipment.
- Ensure compliance with state regulations and Home Instead policies.
- Maintain accurate documentation and client records in accordance with company standards.
Qualifications:
- Current LPN license in the state of Colorado.
- At least 1-2 years of nursing experience, preferably in a home care or senior care setting.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a collaborative team.
- Compassionate and patient-centered approach to care.
- Proficiency in using electronic health records (EHR) systems.
- Valid driver's license and reliable transportation.
