About the Role:
As a Client Care Coordinator, you will be the primary point of contact for our clients and their families, ensuring they receive the highest level of care. You will conduct care consultations, perform quality assurance visits, and work closely with our team to support clients throughout their care journey.
Key Responsibilities:
✅ Conduct care consultations with potential clients and families to assess their needs
✅ Develop individualized care plans that align with clients' preferences and goals
✅ Perform ongoing Quality Assurance (QA) visits to ensure client satisfaction
✅ Communicate regularly with clients, families, and Care Professionals to monitor and adjust care as needed
✅ Collaborate with office staff and the Care Team to maintain a seamless care experience
✅ Address client concerns and problem-solve to enhance service quality
✅ Document client interactions, care plans, and updates using scheduling software

Experience in home care, healthcare, social work, or related fields preferred
Strong relationship-building and customer service skills
Excellent communication, problem-solving, and organizational skills
Ability to multitask and work in a fast-paced environment
