Job Title : Client Care Coordinator

Location : Shelburne, Vermont.

Position Summary : We are seeking a dedicated Client Care Coordinator to join our team. This role involves meeting with potential clients and their families to discuss their needs and developing tailored service plans. The Client Care Coordinator will also conduct service inquiries, care consultations, and coordinate various aspects of client care.
Key Responsibilities :
- Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan.
- Conduct service inquiries and care consultations as needed, following the consultative sales process.
- Work with other team members to coordinate various aspects of a client’s care.
- Conduct client / Care Professional introductions with new clients and new Care Professional shifts.
