Home Instead has been a trusted provider of in-home healthcare services in Seacoast & Southern NH for 23 years, dedicated to enhancing the lives of aging adults with personalized, compassionate care.
We are expanding our clinical team and seeking a full-time Registered Nurse (RN) Case Manager to serve our Seacoast NH territory. This role is essential in ensuring high-quality care by conducting quality assurance visits, collaborating with care professionals, and developing personalized care plans that adapt to clients' evolving needs. The RN Case Manager will work closely with clients, families, and our clinical team to create better outcomes and support independence at home.
Hybrid position - we have a mandatory team meeting that you can attend virtually or in-person in our Portsmouth NH office on Wednesdays. Other times will be case management and planning or traveling to visit clients in-person, in the field across our Seacoast NH territory.
Growth Opportunities – As an expanding company, we have plans to establish franchises in Massachusetts. As the RN for our Seacoast NH territory, you’ll have the chance to grow with us, especially if you already hold or are open to obtaining your Massachusetts nursing license.
Key Responsibilities:
- Travel our Seacoast NH territory to see in-home clients across southern NH towns: Portsmouth, Hampton, Rye, Exeter, Dover, etc. and to our office in Portsmouth for team collaboration.
- Lead case manager for clients and work in collaboration with our clinical team to conduct initial and ongoing holistic assessments of the clients' environment and their medical, social, nutritional, and functional needs.
- Conduct in-person care consultations with clients and families in their homes, rehabs, hospitals, or nursing facilities to finalize client's plan of care and ensure a seamless start of service.
- Build comprehensive care plans in collaboration with clients, families, and the Care Professional team.
- Monitor and evaluate clients' health conditions, ensuring proper documentation with proactive and clear communication.
- Maintain accurate and up-to-date electronic client records and documentation according to company standards.
- Coordinate with healthcare professionals, including physicians, therapists, and other providers, to ensure continuity of care for clients.
- Serve as a liaison between clients, families, and the care professional team, advocating for the clients' best interests.
- Communicate with understanding, building rapport with clients, care professionals and other team members, to successfully resolve open issues to completion.
- Conduct monthly in-person quality assurance home visits to assess client well-being and address any concerns or changes in condition.
- Provide support and guidance to care professionals, offering training and resources as needed to ensure quality of care for clients.
- Provide in-person, hands-on client care when necessary or if needed by scheduling, including ADLs, IADLs, medication management, wound care, and other nursing care in the field.
Qualifications:
- Valid RN license in the state of NH. Licensed in MA NH is a huge plus!
- Minimum of 5 years of nursing experience, preferably in home health or a related setting.
- Experience in case management and care coordination.
- Experience in providing 1:1 care for clients in-home.
- Strong assessment and critical thinking skills.
- Excellent written and verbal communication and interpersonal abilities.
- Compassionate and patient-centered approach to care.
- Ability to work independently and as part of a team.
- Proficiency in Outlook and Excel.
- Valid driver's license.
- All employees must participate in criminal and motor vehicle background checks, as well as urine drug screens.
