Under the direction of the Vice President of Nursing, you’ll lead a team of case management professionals, ensuring residents achieve their highest emotional and psychosocial well-being. You will collaborate with physicians, the interdisciplinary team, and internal and external stakeholders to support smooth transitions during admission, care, and discharge processes.

Position: Director of Case Management
A Day in the Life:
The Director of Case Management directs and oversees trauma-informed case management for residents of the Kosair for Kids Complex Care Center. Under the direction of the Vice President of Nursing, you’ll lead a team of case management professionals, ensuring residents achieve their highest emotional and psychosocial well-being. You will collaborate with physicians, the interdisciplinary team, and internal and external stakeholders to support smooth transitions during admission, care, and discharge processes.
What You'll Do:
- Lead, mentor, and evaluate a team of RN Case Managers, Nurse Navigators, and Social Services Resource Coordinators.
- Oversee medically related case management services, ensuring proper resource and census utilization.
- Collaborate with leadership and physicians to assess prospective residents for admission based on facility eligibility criteria.
- Direct the management of medical records to maintain compliance with federal, state, and local standards.
- Oversee resource utilization and maintain residents' financial eligibility, collaborating with other departments to meet fiscal targets.
- Participate in regulatory inspections and develop plans of correction when necessary.
- Provide case management to support residents and their families in resolving psychosocial and emotional challenges.
- Manage financial resources for residents and serve as a representative payee when needed.
- Build and maintain relationships with referral sources and external providers to ensure continuity of care.
- Lead with trauma-informed values, promoting a culture of inclusion, respect, and non-violence for all stakeholders.
- Ensure compliance with long-term care regulations and maintain professional licensure through continuing education.
- Required:
- Bachelor’s Degree in Nursing (BSN).
- Unencumbered and current Commonwealth of Kentucky Registered Nurse (RN) license.
- Minimum of two years of clinical experience in acute or long-term care.
- Current CPR/Basic Life Support certification (training provided).
- Excellent leadership, interpersonal, and communication skills.
- Strong analytical, critical-thinking, and problem-solving abilities.
- Proficiency with Microsoft Office Suite.
- Preferred:
- Master of Science in Nursing (MSN).
- Experience in case management or long-term care regulations.
