Position: Registered Nurse (RN) Case Manager

Collaborating with physicians and the interdisciplinary team under the direction of the Director of Case Management, you will partner with residents and their families to ensure access to the appropriate resources for maintaining their highest quality of life and well-being. You will play a vital role in ensuring smooth transitions during admission and discharge processes.

Position: Registered Nurse (RN) Case Manager
A Day in the Life:
The RN Case Manager provides trauma-informed, medically related case management to residents of the Kosair for Kids Complex Care Center. Collaborating with physicians and the interdisciplinary team under the direction of the Director of Case Management, you will partner with residents and their families to ensure access to the appropriate resources for maintaining their highest quality of life and well-being. You will play a vital role in ensuring smooth transitions during admission and discharge processes.
What You'll Do:
- Coordinate and facilitate resident admissions, transfers, discharges, and transitions according to regulatory requirements.
- Collaborate with the interdisciplinary team to support residents' medical and psychosocial needs.
- Ensure residents and families have the necessary training and resources before transitions.
- Assess residents to determine appropriate resources, services, and care environments.
- Maintain professional relationships with external providers and referral sources to support continuity of care.
- Assist families with advance care planning, including directives and referrals for palliative or end-of-life care.
- Manage financial resources and act as a representative payee if needed.
- Stay current on long-term care regulations and participate in continuing education.
- Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
- Unencumbered and current Commonwealth of Kentucky Registered Nurse (RN) license.
- Two years of clinical experience in acute or long-term care required.
- Experience in case management or knowledge of long-term care regulations is preferred.
- Current CPR/Basic Life Support certification (training provided).
- Excellent interpersonal skills, empathy, and the ability to build strong relationships.
- Strong critical thinking, analytical, and communication skills.
- Proficiency with Microsoft Office Suite.
