Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls – enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.

We are currently searching for an Advanced Practice Clinician (Nurse Practitioner) to join our expanding house call practice. In this highly autonomous role, you will be responsible for primary care in the home setting in the Philadelphia, PA area. Our house call practice embraces a collaborative style approach that allows you to practice medicine the way you envisioned: patient centered!
Territory includes Bala Cynwyd and Glenside, PA areas
Responsibilities include:
- Perform primary care in the home setting, via Televideo or in partner clinics.
- Drive to patient locations in your assigned geographic area in Philadelphia.
- Average 7-10 patient visits per day.
- Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
- Provide health education to patients. Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
- Provide referrals to appropriate medical resources.
- Utilize our EMR system to document patient visit/interaction accurately and timely.
- Knowledge of HCC coding.
- Active participation in an on-call rotation is required.
- Other duties as assigned.
Qualifications and experience:
- Graduate of an accredited Nurse Practitioner program required.
- Current, unrestricted NP license to practice in Pennsylvania including DEA license.
- Minimum of 2 years of experience as an Advanced Practice Provider in primary care, hospice, palliative or urgent care/outpatient setting.
- Previous experience in a home health, hospice or other home visit setting is highly desired.
- Minimum of 1 year of previous experience utilizing an EMR system with a good working knowledge of HCC coding
- Strong computer skills including MS Office and Outlook is strongly desired.
- Great verbal and written communication skills
- Current drivers’ license, auto insurance and reliable transportation
- Compassionate and caring attitude with a desire to live our mission, vision and values.
