Job Summary
The Home Care LPN is responsible for completing routine assessments of patients in their homes. The LPN will monitor the patient's condition, report any changes, and ensure that care plans are followed.
Key Responsibilities:
- Complete routine assessments of patients to monitor their health status.
- Report any changes in patient conditions to the supervising RN or physician.
- Assist with routine medical procedures and medication administration.
- Document and update patient health records regularly.
- Provide support and education to patients and their families on managing health conditions.
- Communicate with other healthcare team members to ensure coordinated care.
Qualifications:
- Valid LPN license in the state of employment.
- Previous experience in home health care or long-term care preferred.
