We are hiring for a RN Clinical Manager / Field Registered Nurse Hybrid with Home Health Experience.
This is not a remote position, but is a hybrid role where the RN Clinical Manager will work in the office on certain days during the week and the remaining days in the field.
The salary range for full-time is $85,000-$95,000 annually.

About the Role
The Home Health Patient Care Manager is responsible for supervising and coordinating clinical services, as well as providing and directing nursing care to patients in their homes.
- Provides clinical services within the scope of practice, as defined by state laws governing the practice of nursing.
- Receives referrals, ensures clinician assignments, evaluates patient orders, and plots start of care visits.
- Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate.
Requirements
To be successful in this role, you'll need:
- Current RN licensure in the state of practice
- Current CPR certification
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
