What are the responsibilities and job description for the Community Outreach Coordinator position at Homecare COMPANIONS?
Homecare Companions is seeking an energetic Community Outreach Coordinator to serve the Mobile and Baldwin County area. This individual must provide the highest quality service to clients and families with emphasis on creating extraordinary relationships throughout the senior care community. This position requires working independently and as a part of a team. Primary Responsibilities include:
- Build networking relationships through direct outside marketing initiatives with other service agencies and businesses in the senior care arena
- Develop referral streams through community outreach efforts
- Attend business meetings and social events to promote Homecare Companions within the communities of Mobile and Baldwin counties
- Maintain a weekly journal of contacts with established mininum levels of targeted engagement
- Work with other team members and service providers to ensure all aspects of a client’s care are met
- Conduct intake assessments with potential clients and family members to discuss their needs and provide solutions to meet their objectives
- Demonstrate open and effective communication with prospective clients, family members, caregivers, administrative team and referral sources
- Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issues as well as receiving the needed services
- Maintain regular attendance at the office to execute job responsibilities
- Participate in on-call rotation
- Adhere to all company policies, procedures and business ethics codes
- Provide assistance with weekly and daily scheduling as needed
- Fill in as a caregiver when needed to maintain continuous operation and care for clients
- Must have the ability to work in a fast-paced environment
Education/Experience Requirements:
- Certificate or degree in social work, Clinical or Patient Care, or management experience strongly preferred
- At least one-year experience in patient care, health care or senior-related industry required, an equivalent combination of education and work experience may be considered
- Proven outside sales and marketing success-healthcare or senior care strongly preferred
- Must possess a valid driver’s license
- Caring and compassionate attitude
