LVN Nurse
We are seeking a Licensed Vocational Nurse / Licensed Practical Nurse to join our team! You will be responsible for delivering high-quality care to assigned patients. You will be tasked with providing wound-related and other basic nursing care to the patients according to all Wound care Practice Policies and Procedures, Care and Safety Protocols, Clinical Service Practice Guidelines under the direction of a Registered Nurse (RN) or Provider, federal and state regulations, and to the Centers for Medicare and Medicaid Services (CMS) guidelines as applicable.

Responsibilities:

• Gathers patient information and conduct patient examinations
• Under the direction of RN/Provider provides wound-related patient care according to approved and outlined policies/procedures and protocols
• Be able to perform bedside evaluation of the wounds, including focused clinical examination, cleaning wounds and changing dressings
• May provide assistance with case management under the direction of RN/Provider in accordance with state regulation.
• Reports quality of care issues to designated staff
• May function as a documentation assistant in accordance with practice policy
• Perform other duties as delegated

Qualifications:

• High School Diploma or General Educational Development (GED)
• Graduated from an accredited practical nursing program/state-approved educational program
• Current Licensed Practical nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state of practice to maintain throughout the duration of employment in the position
• 1 or more years experience as LPN/LVN to include one or more years in SNF or LTACH setting
• Wound care experience preferred

Required Knowledge, Skills, and Abilities:

• Demonstrated competency in BLS (Basic Life Support)
• Demonstrated prioritization and time management, planning, and computation skills.
• Basic knowledge of regulatory/accrediting requirements for healthcare organizations
• Strong interpersonal, listening as well as oral and written communication skills
• Customer service skills
• Ability to multitask, stay calm in all situations, syndicate and to work in a fast-paced environment
• Problem-solving skills
• Working knowledge of computerized nursing systems
• Be familiar with advanced camera devices

Physical Demands:

• Being in a stationary position for an extended period of time (4 hours or more)
• Viewing computer screen for an extended period of time
• Keying frequently on a computer/tablet for 4 hours or more
• Close distance and peripheral vision
• Lifting/moving item up to 20 pounds
• Pushing/pulling
• Bending/stooping
• Reaching/grasping/touching with hands

Work Environment:

• Normal office environment
• Patient care environment
• Exposure to hazards (bloodborne pathogens, airborne pathogens, body fluids, toxic chemicals, flammable explosive gases, etc.)

Travel Requirements:

• Less than 10% of the time
