- This is a salaried full-time, Monday through Friday, Daytime position.
- This position requires a valid Pennsylvania driver’s license, and reliable internet access.
The Clinical Community Liaison provides marketing for Homeland’s outreach services (Hospice, HomeHealth and HomeCare) by building positive relationships with key referral sources and establishing a person’s eligibility for care provided. These include (but are not limited to) hospitals, skilled nursing facilities, assisted living, personal care homes and physician offices. The Liaison will guarantee continuity of care for patients transitioning from hospitals to home or facilities, by maintaining knowledge of our services and industry developments, and providing informed recommendations to patients, families, and clients. Success is ensured with the Liaison's tactful and compassionate presentation of the realities of processes and financial costs when caregivers, family and friends of patients are facing difficult life events. The Liaison reports to the Director of Marketing, and coordinates care and services with intake teams, clinical and social services managers, and other relevant team members. The Liaison must be a proficient multi-tasker, adaptable, and sensitive to the needs of others.
Requirements Include:
Education
- Clinical: RN, LPN, MSW, or BSW preferred
- BS Business Administration/Marketing/Communications or equivalent work experience preferred
Experience/Skills
- 2 years of clinical experience, within the area of hospice or home health preferred
- 2 years of marketing or liaison experience, within a medical setting preferred
- Good communication
- Computer skills required, with knowledge of Microsoft, EMR and CRM platforms
Licensure, Registration, and/or Certification
- LPN and RN applicants are required to have a current relative license from the Commonwealth of Pennsylvania.
- MSW and BSW applicants are required to have an active LSW from the Commonwealth of Pennsylvania, or a MSW or BSW from a nationally accredited college or university.
- Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania state requirements.
- Satisfactory criminal background check.
- Cardio-Pulmonary Resuscitation (CPR) certified, or ability to do so.
- Must have reliable internet access.
