Job Description

We are seeking a compassionate and caring Certified Nurse Aide (CNA) to join our team at Homeland HomeHealth. The ideal candidate will have a strong desire to help patients with healing and restoration, and possess excellent communication and empathy skills.

Responsibilities

Provide direct personal care to patients in their place of residence according to an established individualized plan of care and under the supervision of a Registered Nurse (RN).
Observe the patient, report, and document observations and care performed.
Provide timely documentation of services based on the patient's care plan.

Requirements

Experience and Education :

Minimum age requirement of 18.

Minimum of one (1) year of direct patient care experience in an institutional or home care setting required, with community health / home care experience preferred.

High school diploma or equivalent.

Completion of a nurse aide training and competency evaluation program approved by the State as meeting the requirements under CMS § 483.151 through § 483.154 and is currently listed in good standing on the State nurse aide registry.

Licensure, Registration, and Certification Requirements :

Valid Pennsylvania CNA Certification.

Valid driver's license and reliable transportation that is insured in accordance with Pennsylvania state requirements and agency policy.

Satisfactory references from employers and / or professional peers.

Satisfactory criminal background check.

Maintains current CPR certification.

Access to reliable internet access outside of the workplace setting.
