The mission of Homeless Childrens Network is to decrease the trauma of homelessness and domestic violence for children, youth, and families; to empower families; and to increase the effectiveness of collaborative efforts among service providers to end homelessness and poverty.

Homeless Children's Network is a community-based organization providing mental health and family support services to children and families who are homeless and formerly homeless.

JABALI YOUTH SUD OUTPATIENT PROGRAM

Jabali Youth SUD Outpatient Program is a culturally responsive and family-centered substance use treatment program for youth in San Francisco. Utilizing Evidence-based and locally-innovative interventions, this program will provide co-occurring or SUD standalone services for youth who are at risk or struggling with drug, alcohol, and other substances. Jabali offers a continuum of services depending on the level of care needed :

Early Intervention for youth who are at risk of developing a Substance Use Disorder (SUD),

Outpatient Level 1 for youth who are actively using substances and meet criteria for SUD diagnosis, but who present with NO acute intoxication and / or withdrawal potential.

Intensive Outpatient for youth who are actively using substances, meet criteria for SUD diagnosis, and who present with acute intoxication and / or withdrawal potential.

This program serves all ethnicities and populations within San Francisco with focused expertise to meet the needs of youth under the age of 18 years old who are Medi-Cal beneficiaries.

POSITION SUMMARY :

In this Drug Medi-Cal program, LPHA plays a hybrid role of clinician and clinical lead for AOD Counselors under the supervision of the Clinical Director. This role involves co-occurring treatment and support of youth and their families. Thus, this role will have direct service tasks as well as some oversight of AOD Counselors work. This position works very closely with the AOD Counselor to serve clients.

The LPHA will assess, determine medical necessity, diagnose, and create the treatment plan for the youth and family in collaboration with the AOD Counselor. As the clinical lead, the LPHA will direct the treatment direction and monitor clients progress in collaboration with the AOD Counselor, the Medical Director, and Clinical Director. They will provide clinical consultation to AOD counselors and other staff in collaboration with the Clinical Director. Thus, regular and frequent care coordination with those entities is expected to provide the best care for the youth and family, but especially with the AOD Counselor on a daily basis.

The LPHA will provide some direct service, while many direct service tasks may be held by the AOD Counselor. The LPHA will solely provide family therapy, do crisis interventions, and lead care coordination meetings with clients other providers and systems. They will especially collaborate with clients mental health and psychiatric team. In collaboration with AOD counselor, LPHA may provide collateral support to family / significant people, lead psychoeducation groups, and do group counseling. Under the supervision of the Clinical Director, the LPHA will do clinical consultation with the Medical Director on addiction matters and exploration of need for medication (MAT) to address substance use.

The LPHA will also engage in some administrative duties. In collaboration with the Clinical Director, the LPHA will support with assigning cases to AOD counselors and will participate in referral triage and follow-ups. The LPHA will also review and approve all work performed and documentation by the AOD Counselor under the direction of the Clinical Director. Additionally, they will review charts to ensure compliance and support AOD counselor in being compliant. Finally, in collaboration with Clinical Director, LPHA will ensure that services comply with county, State, and Federal standards.

Duties and Responsibilities :

  • Assessment and Diagnosis :

Conduct initial assessments to evaluate clients' substance use history, including their emotional, psychological, and behavioral conditions.

  • Use a variety of diagnostic tools, interviews, and questionnaires to identify substance use disorders and potentially co-occurring mental health disorders.
  • Formulate a comprehensive diagnosis based on assessment results, medical history, and client and family feedback.
  • Treatment and Intervention :
  • Develop and implement individualized treatment plans tailored to the client's needs.

  • Offer crisis intervention for individuals in acute distress or emergencies.
  • Use evidence-based practices to support clients through their recovery journey.

    Coordinate with the Medical Director on medication assisted treatment for substance use, as appropriate.

    Monitor clients progress to determine whether client needs to move up or down within the SUD continuum of care

  • Counseling and Support :
  • Offer one-on-one counseling or group therapy sessions for clients dealing with substance use issues.

    Offer family therapy and collateral support to family and significant persons

  • Provide emotional support, coping strategies, and behavioral management techniques.
  • Support clients in addressing personal or social problems that may impact their substance use.
  • Client Education :
  • Educate clients and their families about substance use disorders, mental health disorders, treatment options, and coping strategies.

  • Promote mental wellness and help clients develops long-term strategies for managing their substance use and mental health.
  • Collaboration and Referral :
  • Collaborate with other mental health professionals, including psychiatrists, social workers, and counselors, to provide holistic care.

    Collaborate with other systems, including school, probation, child welfare and primary care.

    Refer clients to specialists for medication management, more intensive therapy, or additional support if necessary.

  • Documentation and Record-Keeping :
  • Maintain accurate and up-to-date client records, including treatment plans, progress notes, and session details.

    Review and approve the AOD Counselors documentation.

    Ensure documentation complies with all relevant legal and ethical guidelines, including confidentiality and privacy standards (e.g., HIPAA).

  • Advocacy and Community Outreach :
  • Advocate for substance use and mental health awareness and reduce stigma in the community.

  • Provide resources and referrals to support services, including community mental health programs, shelters, and support groups.
  • Professional Development :
  • Engage in continuing education, professional training, and supervision to stay current with best practices and new substance use and mental health treatments.

  • Participate in workshops, seminars, and conferences related to substance use and mental health care.
  • Required Qualifications :

    A minimum of 2.5 years working with at-risk youth and their families.

    Prior experience working with individuals dealing with substance use and co-occurring mental health disorders, ideally in a clinical, outpatient, or hospital setting.

    Knowledgeable about the biological and psychosocial dimensions of substance use disorders for adolescents and their treatment, and are able to identify signs and symptoms of acute psychiatric conditions including psychiatric decompensation.

    Experience working and engaging with youth and their families, youth in child welfare system, youth within the juvenile justice system, youth with disabilities, youth with co-occurring disorders, and / or gang-affiliated, etc.

    Knowledgeable about adolescent development; and Experience in clinical treatment, healing arts, and intervention techniques / approaches to youth development.

    Knowledge of youth services in San Francisco from which clients can benefit.

    Knowledgeable and experience with Medi-Cal or Drug Medi-Cal services.

    A Master's degree or higher in psychology, social work, counseling, or a related field (e.g., MSW, MFT, LPC).

    Licensed in the State of California with either Board of Behavioral Sciences (BBS) or Board of Psychology (BOP) (e.g., Licensed Professional Clinical Counselor (LPCC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Psy.D. or Ph.D.).

    Skills and Abilities :

    Strong interpersonal and communication skills, with the ability to build rapport and trust with clients.

    Expertise in various therapeutic techniques

    Ability to diagnose mental health disorders and develop treatment plans.

    High level of empathy and compassion, with a non-judgmental approach to clients.

    Ability to manage sensitive and confidential information.

    Crisis intervention and problem-solving skills.

    Cultural competence and sensitivity when working with diverse populations.
