The Supportive Housing Case Manager at HSI will provide Case Management services to formerly homeless clients currently residing in shared, permanent, supportive housing, utilizing prior knowledge of County and HUD funded resources. This role provides opportunity for an outstanding direct care experience working side by side with our talented staff, reporting directly to the Associate Director of Supportive Housing.

This onsite, full time, hourly, non-exempt position may be eligible for up to one remote work day per week at management discretion.
The Supportive Housing Case Manager will :
Provide crisis intervention, assessment, advocacy and service linkages for residents presenting with behavioral health concerns as part of ongoing supportive counseling and case management support for residents.
Provide in-home services for residents based on individualized needs, which may include : facilitating house meetings, creating and enforcing shared-housing agreements, providing guidance and support for residents as it relates to life-skills and tenancy-skills, and ensuring all basic needs are met.
Appropriately maintain timely, well organized HUD / CoC records, including input in HMIS system of client consent form, intake, contact notes, service plans, annual updates, activity attendance, and discharge.
Work collaboratively with Volunteer Manager and Development Team to plan and execute special events, such as therapeutic and life-skill programming and in-kind donation drives to benefit residents.
Conduct outreach calls and suitability screenings for new program applicants.
Develop and maintain collaborative relationships with other agencies. Represent Homeless Solutions in a positive manner.
Provide as needed guidance and direction for volunteers and interns.
Position Requirements :
Bachelor's degree in Social Work or related discipline required. Experience with residential settings or permanent supportive housing programs is preferred. Persons with lived experience are encouraged to apply.
Experience working with vulnerable populations (i.e. chronically homeless, severely and persistently mentally ill, those with cooccurring disorders, domestic violence survivors, etc.) is required.
Knowledge of Morris County social service resources and affordable housing resources, HUD- funded projects is preferred.
Basic knowledge of and demonstrated experience with administrative functions and Microsoft Office software including Word and Excel.
Valid driver's license with a clean driving record required.
Ability to communicate clearly and professionally, both verbally and in writing.
Regular and predictable onsite attendance and reliable transportation are required during the scheduled hours for this position.

Full time permanent staff of Homeless Solutions are eligible to enroll in fully paid medical, dental and life insurance coverage plans in accordance with HSI policies. Annual compensation starting at $45,750.
