• QUALIFICATIONS / REQUIREMENTS :
• NYS License (Required)
• Basic computer literacy skills
• 1 year Home Health, Outpatient, SNF experience preferred.
• Evaluate and develop a plan of care according to the patients' prior and current functionality.
• Good communication
• between the office staff and therapists.
• Treat patients with therapeutic practices designed to relieve pain, increase mobility, restore normal function, and maintain maximum performance
• Maintain accurate and
• timely
• documentation of individualized care
• Collaborate with clinical staff to develop treatment plans that meet the needs of patients
• Adhere to industry standards and medical guidelines
