Responsibilities:
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop appropriate service plans
- Coordinate and collaborate with other service providers to ensure comprehensive care
- Advocate for clients and connect them with community resources
- Monitor client progress and make necessary adjustments to service plans
- Provide crisis intervention and support during emergencies or difficult situations
- Utilize motivational interviewing techniques to promote behavior change and self-sufficiency
- Assist clients in accessing healthcare services, including Medicare and Medicaid
- Provide guidance and support in workforce development, including job search assistance and resume building
- Offer addiction counseling and support for individuals struggling with substance abuse issues
- Provide mentoring and guidance to clients, promoting personal growth and development
Requirements:
- Bachelor's degree in social work or related field (Master's degree preferred)
- Previous experience in case management or related field
- Familiarity with crisis management techniques and interventions
- Strong communication skills, both verbal and written
- Ability to work independently and as part of a team
- Excellent organizational skills and attention to detail
- Proficiency in using computer systems for documentation and record keeping
- Understanding of behavioral health principles and practices
