Addiction Specialist Provider (Full-Time or Part-Time)

Hometown Health Center (HHC) is seeking a dedicated Addiction Specialist Provider (Physician, Nurse Practitioner or Physician Assistant) to join our office-based, outpatient Medication-Assisted Treatment (MAT) program. This position is available as full-time or part-time and offers a unique opportunity to make a profound impact in the lives of individuals struggling with substance use disorder in Central Maine.

Established in 2003, HHC is a Federally Qualified Health Center (FQHC) and a leader in integrated healthcare. We provide more than 32,000 medical, dental, and behavioral health visits each year to over 8,200 patients — many of whom might otherwise not have access to care.

As we continue to grow, we are excited about the development of our new state-of-the-art facility in Palmyra, which will enhance our ability to provide comprehensive care to our communities.

About the Role

The Addiction Specialist Provider is responsible for delivering onsite, outpatient medical care as part of a multidisciplinary team. This includes assessments, medication management and the development and implementation of individualized treatment plans for patients with substance use disorders. This role works closely with behavioral health providers, care coordinators and medical staff to ensure a patient-centered approach that promotes recovery and long-term health.

Key Responsibilities
  • Provide medication-assisted treatment and monitoring for patients with substance use disorders
  • Conduct medical assessments, including history and physical examinations
  • Prescribe appropriate medications and interventions based on patient evaluation
  • Develop and implement individualized treatment plans in collaboration with the care team
  • Continuously monitor patients’ medical status and adjust care plans as needed
  • Maintain accurate, timely medical documentation according to HHC standards
  • Refer patients to appropriate services when care needs exceed HHC's scope
  • Participate in case reviews and support a team-based approach to care
  • Stay current on best practices and emerging trends in addiction medicine

Minimum Qualifications
  • Licensed Physician (MD/DO), Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Maine
  • Current DEA licensure in the State of Maine
  • Certification or eligibility for certification in Addiction Medicine (preferred but not required)

  • Strong interpersonal and communication skills
  • Ability to work collaboratively in an integrated care environment
  • Comfortable working independently and managing a patient caseload
