What You’ll Do:
As a Family Nurse Practitioner, you will be a vital part of our healthcare team, delivering compassionate, patient-centered care. Your responsibilities will include:
- Health Assessments: Conduct comprehensive physical and psychological assessments through medical history and physical exams.
- Treatment & Care Coordination: Collaborate with physicians and healthcare professionals to determine appropriate treatment plans for your patients.
- Patient Education: Advise and educate patients and their families on their health, treatment options, and progress.
- Community Connections: Coordinate with community resources and make appropriate medical or social service referrals when necessary.
- Telehealth & Home Visits: Provide telehealth services and conduct home visits for patients with medical and dental needs, whether or not they have web enablement.
What You’ll Need:
- Registered Professional Nurse (RN) License in New York State.
- Master's Degree or Graduate Degree from an approved NP program.
- Two (2) years of clinical experience as a Nurse Practitioner preferred.
- Strong Communication Skills: Ability to interact effectively with patients, families, and healthcare teams.
- Telehealth Experience: Familiarity with telehealth services is a plus, but not required.
