Position: Patient Care Navigator
Pay Range: $21.00-27.00
Location: Schenectady, New York
We are seeking a dedicated and compassionate Patient Care Navigator to join our team on a full-time basis, working collaboratively within the patient care team to support all patient care needs.

RESPONSIBILITIES:
- Assists patients in care navigation processes, and psycho-social needs and their support system toward the development of an appropriate treatment plan under the direction of the provider and in coordination with other pertinent allied health professionals.
- Provides patient education programs and tools within the scope of practice.
- Facilitates Transition of Care Management (TCM) by following patients throughout the care continuum and collaboration with inpatient/outpatient care management resources.
- Engages in performance improvement activities i.e., on-going improvement action plan to address quality metrics gap reports.
- Performs outreach to establish and maintain positive intra/ and interagency working relationships.
