Description
The Program Manager provides staff / volunteer management, program development and implementation, community involvement and a safe / clean living environment.
Program & Operations
- Coordinates internal and external resources to enhance the lives of the individuals living at Homeward Bound.
- Manage standard operating procedures within the assigned home(s) to assure compliance with organizational policies / procedures / standards, state licensing requirements, and other applicable laws or provisions.
- Is responsible for the delivery of services for all individuals in assigned homes(s). This includes program development, implementation and data collection.
- Provides financial management for individuals living in the assigned home(s).
- Coordinates and facilitates Interdisciplinary team (IDT) meetings, and ensures comprehensive program plans and risk management plans of each individual in the assigned homes.
- Manages physical and financial resources within the assigned home(s), including household budget, the purchase of supplies and equipment.
- Attends meetings and training as required.
- Completes thorough investigation and reports for internal purposes and external agencies.
- Serve as a liaison to community, case managers, guardians, family members etc. and ensures concerns and issues are promptly addressed / resolved.
- Required to work 20 hours per week direct care (in ratio), including two weekend direct care shifts per month, and until 8 : 00pm at least one day per week.
- Provides human resource management within the assigned home (s), including hiring, training, supervising, performance management, staff development, disciplinary action and dismissal.
- Plans and facilitates employee meetings, initial training, and ongoing training.
Requirements
Must meet one of the following four Minnesota 245(d) minimum standards for a Designated Coordinator :
1) a bachelor's degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children.
2) an associate degree in a field related to human services, education, or health and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children.
3) a diploma in a field related to human services, education, or health and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older or equivalent work experience providing care or education to vulnerable adults or children.
4) a minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
