The Opportunity
We seek a full-time Care Navigator passionate about helping people in rural communities through a centralized telephonic and virtual support model.
In this role, you will serve as a key point of contact for Homeward members, providing telephonic and virtual outreach and support to help them navigate their healthcare needs. You will conduct proactive outreach calls, engage members in meaningful conversations about their health, coordinate access to care, and connect them with medical, behavioral, and social services. By working closely with members, providers, and community resources, you will play a crucial role in ensuring members receive the care they need in a timely manner.

- Build rapport and trust as a telephonic and virtual point of contact for Homeward members, addressing their health-related needs and connecting them to appropriate services
- Conduct proactive outreach calls to build relationships with members, discuss their health status, and encourage engagement in recommended care
- Assist members in scheduling healthcare services, coordinating appointments, and overcoming logistical barriers to care
- Support fulfillment of recommended health services, including obtaining prescribed medicines and arranging for medical, behavioral, and social support services
- Reinforce education provided by Homeward and external providers on preventive care, health literacy, and self-management strategies to empower members in their health journey
- Maintain accurate and timely documentation of all member interactions, navigation activities, and outcomes within the designated electronic platform
- Collaborate with care teams, providers, and community organizations to ensure members receive coordinated and comprehensive support
- Identify and maintain a list of community resources to enhance the support available to members
- Participate in regular staff meetings, training, and professional development opportunities to enhance skills and effectiveness in member engagement
- Completion of a Medical Assistant from an accredited program or school
- High school diploma or equivalent
- At least two years of experience in high-touch, patient-facing or telephonic care roles, preferably with seniors
- Passion for delivering care in rural America and ability to engage members effectively over the phone
- Strong organizational and communication skills to effectively coordinate care and address member needs remotely
- CHW credentials
- Previous experience in a fast-paced, high-growth environment
- Experience working with connected devices/internet-of-things is a plus
