We are looking for a dedicated Care Coordinator to join our team on a part-time basis. This role involves performing key office duties, providing client care coordination, and ensuring that our caregivers deliver the highest quality service to our clients. Key Responsibilities:
General Office Duties: Answer phones, greet clients, manage scheduling, and maintain client and caregiver files.
Care Coordination: Develop, monitor, and evaluate care plans to ensure clients receive top-notch care.
Scheduling: Communicate daily scheduling changes with clients and caregivers, ensuring caregivers work their scheduled shifts.
File Management: Maintain accurate and complete employee and client files, inputting information into the scheduling system.
Coverage Management: Manage last-minute caregiver call-outs and find appropriate coverage.
On-Call Pool Development: Work with HR to develop a pool of caregivers with flexible availability.
Relationship Management: Build and maintain positive relationships with clients, families, caregivers, prospective employees, and referral sources.
Caregiver Matching: Listen to client needs and match them to the appropriate caregiver(s).
Care Plan Review: Ensure caregivers understand and follow client care plans.
Complaint Resolution: Address and resolve caregiver and client grievances promptly and professionally.
Recognition & Motivation: Recognize caregiver performance and promote a positive work environment.
Utilization Management: Monitor and address hours utilization for caregivers as needed.
Supervisory Visits: Perform meet & greets and home visits for caregivers as necessary.
On-Call Coverage: Provide evening and weekend on-call coverage on a rotating schedule.
Ideal Candidate Requirements:
Professional Appearance: Maintain and present a well-groomed, professional image.
Experience: Preferably 2 years in a scheduling or client care coordination role in a homecare agency or similar business.
Caregiving Background: Must have 2 years of experience working as a CNA or caregiver.
Tech Skills: Proficient with Microsoft Office (Word and Excel); willing to learn new scheduling and healthcare applications.
Communication: Strong verbal and written communication skills.
Adaptability: Ability to remain flexible, calm, and resilient in changing situations.
Organizational Skills: Strong ability to prioritize tasks and complete work independently with minimal supervision.
Relationship Building: Ability to develop goodwill with staff, clients, families, and referral sources.
Leadership: Demonstrated ability to support, lead, and motivate a caregiver team.
