We are seeking a part-time nurse to join our homecare team. You will be responsible for opening cases, completing initial assessments, developing a plan of care, and guiding caregivers. You will be required to communicate any changes to the client’s condition in real time to the Client Service Team.

Essential duties and responsibilities:
- Client Care Support:
- Complete initial assessment
- Complete a home visit every 60 days
- Develop Plan of Care
- Complete Caregiver Case Orientation
- Complete Caregiver skill assessments
- Family Follow up as necessary
- Medication follow-ups
- Post Hospital Visit
- Administrative:
- Commission on Accreditation for Home Care (CAHC) Required paperwork
- Basic journal entries
- Discharge Summaries
- Caregiver Performance Review
- Quality of Care Input
- Play an active role in weekly client service roster review meeting
- Be onsite the day new cases open to complete the assessment and orient the caregiver
- Ensure each client is seen every 60 days
- Ensure all administrative tasks are completed on time and with a high degree of detail
- Valid nursing license (Must be a Registered Nurse)
- Strong communication skills to interact effectively with patients, families, and medical staff
- Available to work 20 hours per week
