Job description:
Registered Nurse Case Manager: plans, organizes and directs care for clients living at home. Including completing reauthorizations through social work departments, updating Plan of Cares as client changes occur, act as a care coordinator for their providers in correlation to homecare, and perform tasks delegated to you by D.O.N, and act as a trainer to unlicensed professionals caring for client on your roster.
Field Nursing: preforms private duty nursing to clients in their homes. Example: INR checks, glucose activities, Med set up, wound care, etc.

Job Functions/Responsibilities:
- Completes an ongoing comprehensive assessment of client and family to determine homecare needs. Provides a complete physical assessment and history of current and previous illness.

- discharge/hospitalization/ care coordination
- Main point of contact for roughly 45 clients for nursing duties throughout employment
- Provides professional nursing care by utilizing all elements of nursing process.
- Assesses and evaluates patient's status by:
- writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, participating in revising the plan of care as necessary, initiates the plan of care and makes necessary revisions as patient status and needs change, uses health assessment data to determine nursing diagnosis, initiates appropriate preventive and rehabilitative nursing procedures, administers medications and treatments as prescribed by the physician in the physician's plan of care, counsels the patient and family in meeting nursing and related needs and provides health care instructions to the patient as appropriate per assessment and plan.
- Caregivers will communicate changes in client condition to case managers, follow ups will be conducted and documentation entered into client system.
