Role Overview
As the Care Team Support Specialist, you will be an essential part of our Care Management department, ensuring seamless coordination between caregivers, clients, and office staff. Your expertise in documentation, organization, and communication will help us in maintaining accurate client records, updating service plans, and facilitating smooth operations within the care team. This role is perfect for someone who thrives in a detail-oriented, fast-paced environment and is passionate about making a difference in home-based care.
Key Responsibilities
Client Data Management & Documentation
- Accurately enter and update client information in the management system.
- Maintain and organize digital and physical client records for seamless accessibility.
- Assist in updating service plans by transcribing notes from Care Managers.
Document Preparation & Organization
- Prepare in-home client binders and reassessment documents.
- Scan, file, and organize key documents to ensure regulatory compliance.
- Manage office supply needs to support Care Management operations.
Communication & Coordination
- Answer inbound office calls, directing inquiries and addressing caregiver/client concerns.
- Schedule reassessments and caregiver check-ins to support care continuity.
- Serve as a liaison between caregivers, clients, and the Care Management team.
Administrative Support
- Ensure timely completion and accuracy of client documentation.
- Assist with other administrative tasks to optimize the efficiency of the Care Management team.

Qualifications
- Experience: 1 years in an administrative or operations role, preferably in healthcare or a related field.

- Professionalism: Ability to maintain confidentiality and handle sensitive information with discretion.
