Care Manager Responsibilities:
- Conduct client assessments & develop personalized care plans
- Communicate with clients & families for service satisfaction
- Supervise and support caregivers, ensuring top-quality care
- Maintain compliance with regulatory and agency requirements
- Manage and schedule caregiver shifts for clients
- Screen and interview candidates for caregiver positions
- Coordinate care with other community services as needed
Care Manager Requirements:
- Minimum 3 years experience in home care, home health, or related field
- Excellent communication, organization and leadership skills
- Reliable transportation, valid driver’s license, background check clearance
- Compassionate, positive and empathetic demeanor
- Time management and problem solving skills
- Possess strong computer skills
- Maintain high ethical standards, flexibility, and reliability
