- Develop individualized care plans based on assessment findings
- Coordinate with other healthcare professionals to ensure holistic care for clients

- Document assessment findings and care plans accurately and timely
- Valid RN license in the state of Illinois
- Minimum of 2 years of experience in home health or a similar setting
- Strong communication and interpersonal skills
