Minimum Qualifications:
- Experience: A minimum of 6 months full-time or 1 year part-time experience working with geriatric or chronically ill individuals is preferred. Exceptions made for recently licensed CNAs, HHAs, or those providing companionship and housekeeping only.
- Certifications: CNA or HHA certification is preferred but not required.
- Health and Safety: Proof of current TB testing, ability to safely transfer clients, and compliance with infection control protocols.
- Transportation: Reliable transportation, valid driver's license, and automobile insurance required for client transport.
- Background Check: Must be able to pass a criminal background check (national and state-required).
- Communication: Excellent communication skills for interacting with clients, family members, and team members.
Essential Duties and Responsibilities:
- Care Plan Implementation: Follow personalized care plans to ensure clients receive the appropriate support and assistance.
- Client Support: Provide companionship, meal preparation, housekeeping, transportation, and personal care (bathing, dressing, toileting, feeding).
- Communication and Advocacy: Report any changes in client condition to the Care Manager and advocate for client needs.
- Safety and Compliance: Adhere to safety protocols and infection control practices; notify the office if additional supplies are needed.
- Timekeeping and Documentation: Accurately log hours, tasks, and client status using ClearCare Telephony system.
- Professionalism: Maintain a professional appearance and conduct, adhering to company policies and dress code.
Physical Demands:
- Ability to assist with client transfers and repositioning using proper techniques and equipment.
- Reliable transportation to clients' residences.
