Job Description:
Pay rate $14 to $17 Per Hour (Direct openings for Tega Cay, Lake Wylie, Fort Mill, and Rock Hill)

We are currently hiring for caregiver/CNA/HHA positions. All shifts are available.
Make a difference in the lives of seniors! We're seeking compassionate caregivers to join our team and help seniors age in place gracefully. Provide one-on-one non-medical support and build meaningful relationships with those you serve.
Qualifications and Skills:
- One year of experience (full/part-time) caring for geriatric or chronically ill individuals is required. A CNA/HHA license is a plus.
- Proof of current TB testing
- Excellent communication and documentation skills, with an emphasis on accountability, are essential for effective interaction with patients, families, and healthcare professionals.
- Have a mature, compassionate, punctual, and reliable attitude toward patients in need of assistance.
- If transporting clients, reliable transportation, a valid driver's license, and proof of auto insurance are required.
- Experience using WellSky is a plus, but not required
- Pass a criminal background check via the SC SLED process
- Pass a 10-panel drug screen
Responsibilities
With dignity, compassion, and care, your duties include, but are not limited to:
- Provide personalized care tailored to meet the unique needs and preferences of each client.
- Monitor clients' health conditions and report any changes to the healthcare team to prevent rehospitalizations.
- Implement fall prevention strategies and ensure a safe living environment for clients.
- Engage clients in life enrichment activities to enhance their quality of life and promote well-being.
- Foster strong relationships with clients and their families, demonstrating empathy and compassion.
- Ensure timely and accurate completion of all care-related tasks and documentation.
- When possible, promote socialization and encourage clients to participate in community and social activities.
- Adhere to established care plans and follow guidelines set by healthcare professionals.
Daily Duties May Include
- Personalized Care: provides assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and ambulation.
- Medication Reminder: Provide timely medication reminders, strictly adhering to prescribed dosages and safety protocols. Report any discrepancies or concerns immediately.
- Meal Planning and Preparation: Prepare nutritious meals that meet the client's dietary needs and preferences, promoting healthy eating habits.
- Light Housekeeping: Perform light housekeeping tasks, such as laundry and cleaning, to maintain a clean, safe, and organized living environment for the client.
- Fall Prevention: Proactively identify and mitigate fall hazards in the client's environment, adhering to established protocols and conducting regular safety assessments.
- Rehospitalization Prevention: Monitor client health, promptly reporting changes to the care team to prevent rehospitalizations. Follow post-discharge instructions and coordinate with the care team.
- Life Enrichment and Socialization: Engage clients in stimulating activities tailored to their interests to promote socialization, cognitive function, and emotional well-being.
- Punctuality and Reliability: Be punctual and consistent, providing reliable care and building trust with clients through timeliness.
- Communication: Document and report all changes in client condition, concerns, or incidents promptly and accurately, maintaining open communication with clients, families, and the care team.
Job Expectations:
- Must be able to lift 50 lbs. and assist clients with physical needs, including walking and turning.
- Must have reliable transportation (proof of insurance required) and be able to serve clients in Tega Cay, Lake Wylie, Fort Mill, and Rock Hill.
