Role Overview

As a Lead Caregiver , you will play a crucial role in delivering high-quality care, mentoring caregivers, and ensuring that client needs are met with professionalism and compassion. You will be responsible for ensuring the accuracy of care plans, conducting re-assessments, and maintaining compliance with regulatory and agency standards. This role is ideal for a seasoned caregiver looking to step into a leadership position while continuing to make a direct impact on client care.

Key Responsibilities

Client Care & Advocacy

  • Deliver compassionate, high-quality care to clients while ensuring their dignity and independence.
  • Conduct client re-assessments and assist in the development of personalized care plans tailored to individual needs.
  • Educate clients and families on available services, empowering them to make informed decisions.
  • Serve as a point of contact for clients and their families, addressing concerns and ensuring satisfaction.
  • Caregiver Coaching & Support

  • Provide on-site feedback and coaching for caregivers, ensuring they follow care plans and best practices.
  • Conduct caregiver introductions for new clients, setting clear expectations for service delivery.
  • Monitor and assess caregiver performance, providing feedback and additional training as needed.
  • Provide feedback for personnel evaluations, recognizing top performance and identifying areas for improvement.
  • Care Quality and Coordination

  • Ensure care plans are up-to-date, accurate, and aligned with client needs.
  • Audit care logs to ensure accurate caregiver shift completion.
  • Conduct scheduled and unscheduled in-home visits to assess service quality.
  • Report and investigate any client concerns or caregiver performance issues, implementing solutions promptly.
  • Maintain clear communication with office staff, including scheduling, care management, and HR teams.
  • What Success Looks Like

    You are the top caregiver : leading by example when mentoring caregivers and interacting with clients.

    You maintain a high quality of care by consistently conducting re-assessments and updating care plans to ensure their accuracy.

    You are passionate about client and family satisfaction, proactively solving and communicating problems to relevant stakeholders.

    Qualifications

  • Experience : Minimum 2 years in home care, home health, or a related field.
  • Skills :
  • Strong written and verbal communication to interact effectively with clients, caregivers, and professionals.
  • Ability to supervise, evaluate, and support caregivers in a leadership capacity.
  • High attention to detail with impeccable follow-through.
  • Problem-solving mindset with the ability to exercise sound judgment.
  • Requirements :
  • Reliable transportation, valid driver's license, and current insurance.
  • Must pass a criminal background check.
  • Must pass pre-employment physical and TB screening.
