Job Summary:
Serves as the liaison between the resident and other staff, resident’s family and the community through direct and indirect supportive services under the supervision of the Social Services Coordinator.
Essential Functions:
- Treats all information about residents, their condition, and family as confidential information.
- Complies with established Corporate and Departmental policies and procedures, and maintains established standards and practices.
- Performs direct services to residents on a one-to-one basis and groups of residents as directed by the Social Services Coordinator.
- Establishes and maintains cooperative relationships with referral and support services in the community.
- Interviews and provides tours to prospective residents and their family/representative(s) reviewing the variety of services offered by Homewood.
- Prepares prospective residents for placement in accordance with Homewood’s policies and Federal and State laws.
- Maintains an up-to-date social service report on each resident including a social history and quarterly evaluation of social needs.
- Participates with resident care plans, Medicare review, and completion of MDS’s for Nursing Care residents as directed by the Social Services Coordinator.
- Provides a basic orientation to the facility for the resident and family/representative(s).
- Coordinates goals to meet psychological and social needs of residents as directed by the Social Services Coordinator.
- Performs other functions as directed by the supervisor.
Qualifications:
- A Bachelor’s degree from an accredited college in the social services/related field preferred.
- One year experience in long term care working with geriatric persons.
- Ability to read, write and understand English well.
- Has compassion, understanding and empathy for older persons.
- Ability to convey a professional image to the residents and public.
- Ability to identify psycho-social needs of residents, perspective residents and families.
- Ability to communicate to residents at a level they can understand.
- Has an understanding of Medicare/Medicaid regulations as well as basic nursing terminology.
- Must have qualities of initiative and dependability.
