Our compassionate and established Home Health Agency is seeking a Full-time Registered Nurse (RN) to join our team, providing in-home visits to clients in the Indianapolis, IN area.

We are providing Home care services for the residents in the community that require support with ADL's (assisted daily living). Our certified Home health aides are qualified assist in enhancing the lives of the individual.
The RN must be licensed in the state of IN. Must be able to reliably commute to our clients in the area.

Qualifications:
- Current and Valid RN license in IN
- Current and Valid Physical and TB test for up to one year
- IN Driver's license and reliable transportation
- Nursing experience (1yr minimum) (Required)
- Home care experience (1yr) (Preferred)
Responsibilities:
- Complete an initial assessment of patient and family to determine needs.
- Provide a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluate patient nursing needs.
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Use health assessment data to determine nursing diagnosis.
- Develop a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Provide health care instructions to the patient as appropriate per assessment and plan of care.
- Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient.
