JOB DESCRIPTION:
Under the general supervision of the Supportive Housing Program Manager (SHPM), the Housing First Case Manager (HFCM) is responsible for the provision of supportive and intensive case management and permanent housing location for individuals experiencing chronic homelessness. Referrals come directly from Safe Options Support (SOS) and ACT (Assertive Community Treatment) Teams in Orange and Dutchess Counties. The Housing First Case Manager will conduct assessments, develop individualized and person-centered care plans, link homeless individuals with resources, conduct home visits four times a month and provide case management services in conjunction with community partner agencies. The CM will work with coworkers, supervisors and staff from other Agencies to ensure client needs are being met and to improve processes and services. Must be flexible with schedule to meet the needs of the clients which may include some evenings and weekends. Additional hours may be required to meet program deadlines or client needs.
QUALIFICATIONS FOR THE POSITION:

1. Bachelor’s Degree in Human Service related field; with 2-3 years Human Service experience preferred
2. Ability to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people.
3. Integrity, positive attitude, mission-driven, and self-directed with demonstrated passion for HONOR’s mission.
4. Detail-oriented and have competent literacy and writing, documentation, communications and interpersonal abilities (friendly, courteous, helpful, ability to work as part of a team).
5. Ability to function independently and have flexibility, personal integrity and the ability to work with residents, staff and support agencies.
6. Experience using MS Outlook, Excel, and electronic health record
7. Bi-lingual preferred
8. Valid New York State driver’s license
