We are seeking a dedicated Hospice RN Case Manager to join our team. In this role, you will play a crucial part in assessing and managing the care needs of patients with terminal diagnoses, while guiding and supporting both patients and their families throughout their end-of-life journey. Hospice RN Case Manager Responsibilities:
- Conduct initial and ongoing comprehensive assessments of the impact of the terminal diagnosis on patients' physical, functional, psychosocial, and environmental needs, as well as their activities of daily living. Assessments also include evaluating the risk for pathological grief, cultural and spiritual considerations, and communication patterns.
- Develop and implement individualized care plans, revising them as needed to address changes in patient condition and provide relief from discomfort and symptoms through specialized palliative and end-of-life care.
- Serve as a resource and provide education to patients and families regarding the disease process, self-care strategies, end-of-life care, and ethical concerns.
- Initiate and manage preventive and rehabilitative nursing procedures tailored to patient needs.
- Prepare clear and detailed clinical and progress notes, ensuring they reflect progress toward care goals.
- Coordinate and prioritize patient and family services in collaboration with the interdisciplinary team, ensuring a holistic approach to care.
- Utilize case management principles, referring patients to additional services as appropriate.
- Communicate changes in patient status or needs to the physician and other team members, ensuring timely adjustments to care plans.
- Determine the scope and frequency of services based on patient acuity and family needs.
- Supervise Licensed Practical Nurses (LPNs) and hospice aides in accordance with regulatory guidelines.
- Participate in and contribute to in-service education programs, ensuring continuous professional development.
- Provide hospice-specific training to staff, family members, and caregivers to ensure comprehensive and compassionate care.
- Continuously evaluate patient and family responses to care and adjust plans as necessary to meet evolving needs.
- Assess caregiver ability to provide necessary care upon admission and throughout the care journey.
- Identify personal support needs and utilize available resources to maintain your own well-being as a caregiver.
- Apply hospice care admission and re-certification criteria to ensure appropriate care levels and patient eligibility.
- Effectively communicate and document information using current technology and systems.
- Participate actively in hospice performance improvement initiatives to enhance care quality and patient outcomes.

Requirements
Required Education, Training, and Experience for Hospice RN Case Managers:
1. One year of nursing experience, preferably in Hospice, Home Health, or a Skilled Nursing Facility
2. Unencumbered Indiana License as a Registered Nurse
3. Current CPR certification (Basic Life Support)
Relationships:
Reports to the Hospice Team Leader/Director of Clinical
Qualifications and Requirements for Hospice RN Case Managers:
1. Demonstrated ability to read, write, and speak English.
2. Must possess good verbal and written communication skills
3. Must be able to meet deadlines, work under limited supervision.
4. Must be able to perform cardiopulmonary resuscitation and relief of an obstructed airway;
5. Must be able to climb at least 2 flights of stairs and be able to stand/or walk for a period of at least 4 hours.
6. Valid state driver’s license
7. Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers.
8. Proof of motor vehicle insurance.
Salary : $75,000 - $85,000
