This role is a specialized clinical salaried role that involves the following responsibilities
LPN required/RN preferred
Active Indiana nursing license
Nursing degree from accredited school of nursing
Expert in ensuring qualifying supportive documentation needed to validate the MDS is present in the chart per Supportive Documentation Guidelines
Through knowledge on how to implement, maintain and sustain Nursing Restorative Program with all needed components and supportive documentation required

This role is a specialized clinical salaried role that involves the following responsibilities
- Through knowledge of RAI process including components that Govern both the Federal and State Regulations with various payors
- Through knowledge of PDPM guidelines and clinical areas that affect reimbursement under each of the components
- Assists in the development, implementation, and evaluation of patient specific comprehensive care plans.
- Coordinates the review and revision of the resident's care plan by the interdisciplinary team during required care plan meetings on a quarterly basis, as significant changes occur, and as needed.
- Communicate and collaborate with interdisciplinary team about the identified needs of resident care that would require initiation of a Significant Change
- Accurate and thorough completion of the Minimum Data Set (MDS), Care Area Assessments (CAA's) and Care Plans, per current Federal and State Regulations and guidelines that govern the process.
- Completion of resident assessment and accurate documentation in accordance with state, federal, and facility regulations.
- Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission.
- Prepare and transmit electronic charting and reports for Medicare.
- Ensuring the accuracy of medical records and coding procedures.
- Identify areas of risk and utilize critical thinking skills.
- Thorough knowledge on what components of the MDS affect a 5 Star rating for the facility and how to monitor these components.
