We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare team.
Duties
- Provide direct patient care, including administering medications and treatments as prescribed by physicians.

- Monitor patients' vital signs and report any changes to the healthcare team.

- Educate patients and their families on health management, including health coaching and memory care strategies.
- Ensure compliance with HIPAA regulations to protect patient confidentiality and privacy.
- Collaborate with interdisciplinary teams to provide comprehensive care in acute care settings.
- Maintain accurate patient records and documentation.
Skills
- Knowledge of Hospice MD
- Strong organizational skills to manage multiple tasks efficiently while maintaining attention to detail.
- Excellent communication skills, both verbal and written, for effective interaction with patients, families, and healthcare teams.
