Job Summary: As a Case Manager, you will serve as a dedicated advocate for residents of Hope Christian Services. Your primary responsibility is to ensure their well-being and promote their independence, respecting their rights, dignity, and individuality. This role emphasizes collaboration with all departments to provide a holistic and high-quality residential experience.
Job Qualifications:
Education: Bachelor's degree in special education, psychology, social work, or a related field.
Work Experience: At least two years of relevant experience in the field.
Physical Demands: Capable of independently lifting up to 35 pounds.
Licenses: Valid driver’s license from the state of New Jersey and a clean driving record.
Communication Skills: Exceptional proficiency in written and spoken English, with strong communication skills to engage effectively with residents, families, and all departments.
Responsibilities:
Responsibility to Residents:
Champion the rights, dignity, privacy, and respect of each resident.
Encourage and support the independence of residents through personalized goal setting and training.
Prepare and submit monthly IHP schedules for residents, coordinating scheduling with fellow case managers.
Organize, facilitate, and engage in Individual Habilitation Plan Meetings and Interdisciplinary Team Meetings, in collaboration with families and staff.
Ensure the timely completion of Individual Habilitation Plan Meetings.
Develop individualized IHP goals and objectives, focusing on areas such as self-help, mealtimes, and communication.
Formulate behavior plans in accordance with Hope Christian Services and State guidelines for specific residents, and diligently oversee data collection. Provide training to staff regarding these plans.
Complete and submit required paperwork.
Ensure resident funds are within allowable thresholds.
Regularly monitor the progress of IHP goals.
Schedule, participate in, and complete pre-admission and 30-day assessments for new residents entering group homes.
Voucher resident pharmacy bills and other allowable expenses.
