We are currently seeking a Bilingual Nurse Care Manager to join our team. This role is crucial in supporting our Spanish-speaking clients by providing specialized primary care, nursing services, and care management. About the Position
As a Bilingual Nurse Care Manager, you will play a vital role in helping Spanish-speaking clients navigate their journey to recovery. You will provide essential nursing services, including assessments, care coordination, and medication management, ensuring that each client receives tailored and compassionate care. Key Responsibilities
- Deliver integrated primary care and mental health services, focusing on Spanish-speaking clients.
- Collaborate with the treatment team to develop and implement individualized treatment plans.
- Conduct medical evaluations, crisis interventions, and screenings, prioritizing high-risk conditions.
- Provide education on medication management, chronic disease prevention, and healthy living.
- Coordinate care with external providers and ensure accurate documentation in Health Information Systems.
- Advocate for clients access to necessary benefits and services, including Medicaid and third-party insurance.
- Engage in community outreach to build strong, culturally sensitive relationships with clients and their families.
