What are the responsibilities and job description for the Community Living Supervisor - Emmetsburg position at Hope Haven Inc.?
PRIMARY DUTY
Supervision of staff, including scheduling, hiring, firing, and training. Conducting staff and client meetings as needed. The supervisor is also responsible for staff implementing client programs, following Hope Haven policy and procedures, and ensuring person centered choice within our community living sites.
DUTIES AND RESPONSIBILITIES:
Staff supervision:
Assist with hiring, orientation, and training of all community living staff. Ensure that all hiring and training requirements are met.
Ensure staff work schedules are complete, making adjustments as indicated by programming needs.
Supervise and evaluate Community Living Lead and DSP’s according to their performance.
Work a minimum of 10 to 20 hours per week, may include 1 weekend shift, in one or more of the sites at Manager discretion. This time should be spent directly working with staff and clients to observe, train, and model appropriate work skills, techniques, and documentation. Fill in shifts when needed.
Ensure the program meets all applicable standards including agency, state, and federal regulations, as well as accrediting agencies.
Ensure that all personnel files are kept up to date and accurate.
Develop an environment that maximizes each person’s choice and ability in all daily routines and living skills.
Implementation of Service Plan:
Supervise implementation of the community living person centered program and assure quality services.
Ensure all records are kept up to date, including but not limited to, Credible, medical information, demographic information, progress reports, mileage logs, vehicle safety checklists, EMARS, and financial records in coordination with CL Lead.
On call for emergencies and concerns.
Follow all safety procedures and promote safety among employees and persons served.
Regular and reliable attendance is required.
QUALIFICATIONS:
High school graduate or ability to obtain GED/HSED; BA or BS degree in Human Services field and/or 4 years' experience in Human Services are preferred.
Good communication (verbal and written), organizational and leadership skills.
Computer literate or have basic computer skills.
Clearance from all applicable background checks as required by service standards.
Reliable Transportation.
Provide proof of state minimum liability insurance coverage.
