Job Summary
Summary
Under the supervision of the Administrator, provides direct supervision to home health staff. Ensures that visits and all clinical matters are made in accordance with home health agency policy. Assists with hiring of qualified home care staff. Ensures that policies and procedures are kept current. Interacts with the Professional Advisory Group to provide program evaluation data and policies matters. Occasionally may be required to perform skilled nursing assessments. Hiring and terminating clinical staff.
Major Duties/Responsibilities
1. Responsible for program operations and planning.
2. Evaluates the program, services and field/ clinical staff personnel.
3. Establishes and maintain effective channels of communication.
4. Keep the program personnel up to date with clinical information and practices.
5. Assures adequate and appropriate staffing.
6. Oversees staff development including orientation, in-service education, training and continuing education.
7. Coordinates with other program areas and management as appropriate.
8. Maintains prevalent local issues and trends.
9. Assures that appropriate service policies are developed and implemented.
10. Directs staff in performance of their duties including admission, discharge, and provision of service to clients.
11. Ensures that policy and procedures are kept current with changes in home health care agencies.
12. Ensures that treatment plans are signed and returned to HHHC, LLC. in a timely manner.
13. Follows up with physicians as needed.
14. Supervises Home Health Care RN Case Managers, Home Health Aides and other Home Health Care Providers.
15. Assists with the development of treatment plans.
16. Follows up with RN Case Managers to ensure that care is provided in accordance with the prescribed treatment plan.
17. Ensures that Home Health Care staff is current with continuing education.

Qualifications
Minimum Qualifications
Minimum Qualifications
· Graduate from an approved school of professional nursing
· Current, valid RN licensure with Ohio Board of Nursing
· Valid current automobile insurance coverage
· Two years home health administrative experience
· Three years home health clinical and clinical supervisory skills
- Previous experience as a Care Coordinator or in a similar position is preferred
- Certification as a medical assistant is preferred
- Strong problem-solving and organizational skills
- Ability to manage multiple projects or tasks and prioritize appropriately
- Ability to work in fast-paced situations and make sound decisions quickly
- Excellent interpersonal skills and high level of compassion
- Strong verbal and written communication skills
Compensation: $25.00 - $35.00 per hour
Salary : $25 - $35
