Role:Key Responsibilities:
• Attend the IDT meeting.
• Oversee team expenditures and payroll/overtime, ensure that team members are scheduled in an appropriate manner to limit the use of overtime that meets the needs of the participant.
• Provides utilization reports of services rendered, scheduled, requested along with department expenses.
• Maximize the use of the EHR (PACE Logic) for participant scheduling and utilization reporting.
• Ensure timely documentation of assessments, notes, and reporting requirements within the EHR by the team.
• Ensure quality care is delivered as ordered. Will monitor quality of services using a combination of in-person supervision, participant & caregiver interviews, home care order monitoring and tracking.
• Provide home care assessment in collaboration with the center RNs. Assessments may include but not limited to, initial, semi-annual, change in status, service determination.
• Conduct assessments for all participants receiving home care to determine skilled nursing needs, personal care assistance needs, and homemaker/chore services.
• Oversee home care coordinator schedulers to assist with problem solving around missed events or visits. Ensure timely communication of conflicts with appropriate discipline (transportation, RNs, MSS. Clinic)
• Update the participant’s individualized care plan as appropriate.
• Carry out supervisory responsibilities in accordance with Hope’s Leadership Guidelines. Conduct training of employees; planning, assigning, and directing work; appraising performance; rewarding, coaching, and guiding employees; addressing complaints and resolving work-related issues successfully. Provide stewardship of organization resources.
Knowledge & Skills:
• Excellent communication; via phone, email, and in person.
• Must be flexible to work in other locations if needed.
Required qualifications:
• Must have an active Florida RN License.
• History of effectively managing clinical teams including both exempt and non-exempt employees.
• Previous clinical home care work and supervision preferred.
• Minimum of 1 year experience working with the frail and elderly in a healthcare setting.
• Valid Florida Drivers’ License with good record

• Clear level 2 background check
