Job Summary
The Community Mental Health Worker/Practitioner plays a vital role in helping people with mental illness live in the community in the least restrictive setting, function at their best level, and stay hospital-free. This position assists adults who have major depression, bipolar disorder, schizophrenia, schizoaffective disorder, or borderline personality disorder. The CMHP will teach mental health skills, support the participant's life goals, and help to remove barriers to mental health recovery, while working in participants' homes, in the community, and in the Hope House drop-in center. The ideal candidate will possess a strong understanding of behavioral health and person-centered care, demonstrating compassion and commitment to improving the lives of those they serve.
Duties
- Assess client needs in mental health, financial, housing, transportation, social, benefits, employment, family, etc. and problem solve with participant and team
- Use ARMHS-approved interventions, teach mental health recovery skills, and help resolve barriers to mental health recovery
- Gently intervene as needed with others in participant's life to ensure participant may continue to pursue goals
- Use education and problem solving to help participant reduce stressors and conflict, develop natural supports, and increase resources
- Make home and community visits to accurately assess and assist with goals
- Coordinate/consult with Hope House staff, and multidisciplinary teams
- Use the electronic health record to complete documentation and in a timely manner
- Respond to participant emergencies and crisis situations per policy.
- Introduce participants to groups and activities to develop social supports
- Conduct data collection to monitor participant progress, and identify areas for improvement.
- Maintain confidentiality of participant information in accordance with HIPAA regulations.
- Advocate for participants’ rights and needs within the healthcare system.
- Complete 30 hours of ARMHS-related training every two years
- Transport participants as needed to reduce barriers to life goals. Help participants learn about and use available transportation as well.

Qualifications
- Four year degree or master's in psychology, social work, applied psychology or related field. (If within one year of graduation, see Hope House for internship or other in-training position)
- Experience working with individuals with major depression, bipolar disorder, schizophrenia, schizoaffective disorder, and borderline personality disorder helpful, but not required
- Persons with lived experience in mental health and peer support certification invited to apply. Certification may be gained while working at Hope House.
- Ability to establish a trusting, supportive, healthy atmosphere
- Acceptance of staff and participant diversity, including knowledge and experiences in Ojibwe language and culture
- Able and willing to follow HIPAA regulations and guard participant confidentiality.
- Function cooperatively in team settings and with agencies serving mutual participants
- Maintain policy-based boundaries with participants, coworkers, and other agencies
- Express needs and concerns, listen with understanding and compassion, and engage effectively with diverse groups
- History of documentation skills
- Ability to role model health and wellness
- Must have a current driver's license, vehicle, insurance, and agreement to annual vehicle background checks
Job Type: Full-time
Pay: $35,620.00 - $45,426.00 per year
