Case Manager β Hope Lodge (INFINITI GROUP, LLC)
Hope Lodge provides addiction and mental health treatment through Inpatient Detoxification and Residential Treatment. We prioritize respect and compassion, adhering to the highest ethical standards set by the California Department of Health Care Services and The Joint Commission.
Position Overview:
The Case Manager provides direct services to patients and families, connects clients to community resources, and advocates for appropriate care throughout the recovery process.
Key Responsibilities:
- Provide individual and group counseling, referrals, and aftercare planning
- Coordinate services with community and referral sources
- Assess needs for mental health, substance abuse, and program services
- Complete and update treatment, discharge, and aftercare plans adhering to required timelines
- Assist in the Utilization Review process
Qualifications:
- Preferred: Certified Alcohol and Drug Counselor (CADC, CATC)
- Required: RADT, RAC, SUDRC
- Experience in chemical dependency or behavioral health case management
- Preferred: ASAM knowledge and KIPU experience
- CPR/First Aid certification and TB test required
