Our mission is to bridge the gap in mental health services, offering support to those navigating complex challenges, including stigmatized diagnoses such as personality disorders, trauma, and houselessness.
At Hope Mountain, we take a holistic approach to mental health, and we believe that integrating case management with therapeutic care is essential to achieving long-term success for our clients. As we continue to grow, we are looking for a dedicated Case Manager to join our team and support our Medicaid clients as they navigate the complex landscape of mental health care and benefits.
As an Independent Case Manager at Hope Mountain, you will work closely with clients to support their mental health needs while assisting them with Medicaid-related services. This position is flexible and PRN (as-needed), allowing you to set your own hours and work remotely. Your core responsibilities will include:
- Care Coordination: Collaborate with clients, therapists, and external providers to create and manage personalized care plans that support both mental health treatment and Medicaid benefits.
- Medicaid Benefits Applications & Management: Assist clients in navigating the Medicaid application process, managing their benefits, and ensuring they have access to the care they need.
- Client Support & Advocacy: Provide ongoing emotional and practical support to clients, helping them overcome barriers to accessing mental health care and social services.
- Research & Guidance: Stay up to date with Medicaid policies, healthcare services, and community resources to provide informed advice to clients.
- Progress Tracking & Reporting: Monitor clients’ progress and needs, ensuring that they are receiving appropriate services and that their care plans are adjusted as necessary.
Key Qualifications:
- Experience: Prior experience in case management, mental health services, or social work is required. Familiarity with Medicaid systems and benefits is a plus.
- Knowledge: Understanding of mental health care, community resources, and Medicaid eligibility processes.
- Compassionate & Client-Centered: A passion for supporting individuals with mental health challenges, including those with complex needs or difficult diagnoses.
- Strong Communication: Excellent verbal and written communication skills, with the ability to collaborate with clients, their families, and healthcare providers.
- Self-Motivated: Ability to work independently, set your own hours, and manage a caseload effectively.
- Tech-Savvy: Comfortable using telehealth platforms, case management software, and other digital tools.
- Education: A minimum of a bachelor's degree in the social sciences is required.
