Role Overview
The LPN Shift Team Leader is a crucial part of our multidisciplinary team, responsible for delivering high-quality medical care by overseeing the medical needs of residents and monitoring the operation of the med room.
This role involves modeling and teaching exemplary care to floor staff while coordinating care needs with external medical entities.
Main Responsibilities:
- Treatment: Assist nursing teams in dispensing, monitoring, procuring, and evaluating the efficacy of client medications, including intermuscular injections.
- Treatment: Collaborate with nursing teams to monitor and treat chronic and acute medical conditions within the scope of practice.
- Leadership: Pass medications and facilitate supervision of medication passes when needed.
- Leadership: Provide training and hands-on assistance in residents' activities of daily living and self-care tasks.
- Coordination of Care: Act as liaison with psychiatrists, external medical providers, guardians, residents, and staff.
- Coordination of Care: Work closely with pharmacies and staff regarding medication orders.
- Coordination of Care: Participate in the development and implementation of individual treatment plans for residents.
