Job Summary:
The ideal candidate will have experience in providing medical treatment services within specialized rehabilitation settings and prior mental health experience is preferred. As a key associate, you will play a critical role in ensuring the quality and efficiency of nurse-related operations.
- Responsibilities:
- Facilitate and/or coordinate medication administration and direct individual health care programs for residents.
- Provide education to direct care staff and residents on relevant health care areas as needed.
- Proactively monitor and treat chronic and emergency medical conditions and illnesses under the advisement of affiliate medical providers.
- Develop healthcare goals in collaboration with Case Holders to ensure inclusion in Individual Plans of Service (IPOS) when necessary.
- Act as a general consultant on medical and health-related issues affecting residents.
- Assess resident health needs through admission and annual nurse and health assessments.
- Document medical treatment and health status in resident records for continuity of care and treatment team reference per regulatory standards.
- Communicate relevant medical issues with affiliated medical providers and applicable program management.
- Provide leadership, support, and direction to staff to facilitate communication, problem-solving, and program development.
- Ensure medical compliance with all regulatory bodies.
- Participate in the appropriate nurse "on-call" rotation.
Requirements:
- Experience in providing medical treatment services within specialized rehabilitation settings.
- Prior mental health experience is preferred.
- A valid driver's license is required according to Hope Network policy.
